Rehabilitation and reablement care at home

With our proactive rehabilitation and reablement support, life returns to normal sooner after illness, injury or surgery. Working as part of a team with medical professionals such as physiotherapists, nurses and occupational therapists, our trained carers can provide tailored care to help you manage at home and get back to normal life faster.

Recovery in the comfort of your own home

For many people, being at home with one-to-one care after an operation or illness makes recovery easier and faster than in a residential care setting. You’re in a familiar environment, and can focus on getting better rather than on adjusting to new routines and surroundings.

Personalised care plans

We will meet you and liaise with the hospital discharge team and other professionals involved in your care to develop a care plan that helps you recover as fast as possible. Every carer who visits will know exactly what is agreed and what you want and need. Your care plan could include a mixture of home help support, personal care, and companionship.

Making sure you don’t do too much too soon

When you come home after a hospital stay, it’s important to allow yourself time to rest. A little extra help around the house and with personal care such as showering can make all the difference, keeping you comfortable and making sure you don’t do too much too soon.

Help to move and exercise

Where you have been given exercises and advice about increasing movement, our carers can provide encouragement and support. They will help you build up your confidence and activity levels in line with the specialist advice, so you can maximise your capabilities and not lose mobility for the long term.

NHS reablement packages

If the NHS has given you an intermediate or reablement care package, you may find our care service included. We are happy to keep coming after the NHS package ends – just get in touch to find out how we can help. You may be able to get social care funding towards the cost.

Adjusting to life-changing injuries

If you are returning home after life-changing injuries, our carers can be a lifeline. Trained in supporting people with disabilities, they’ll provide both practical and emotional support as you relearn skills and rebuild confidence. They will work with you to find new ways to manage day-to-day tasks, to set goals, and to get back to the life you once had.

Flexible care arrangements

You can book our carers for whatever care visit arrangements suit you best, from a few short visits through to overnight stays and short term live-in-care. We’re also happy to agree a pattern of visits that changes as you recover - you might want longer or more frequent visits for your first few days at home, for example.

What reablement care costs

The cost of reablement care depends on exactly what you need. We will discuss options and give clear pricing so you can decide what will work best for you. We can also advise on social care funding.

Arrange a care consultation

The best way to see how our care visits could work for you is to arrange for a free no-obligation care consultation. One of our senior staff members will visit you at a time that is convenient to you and discuss your care and wellbeing needs, and advise on the most suitable options and costs.
